Family Tuition Plan Application

We are pleased to announce that the 2018-2019 Family Tuition Plan application is now available online at www.assumptionhigh.org/familytuitionplan. The application includes a drop down menu which will allow translation to Vietnamese and Spanish.

The application form is very similar to previous years, and continues to require a copy of your 2017 Iowa 1040 tax return and Schedules C, E, and/or F if they are a part of your return. You will be asked to upload these documents to the application (be assured that all transmissions are secure). If you did not file an Iowa 1040 for 2017, the online app will indicate how you should proceed.  

PLEASE NOTE: The application must be completed in one sitting, so be sure you have everything you need ready before beginning the application process.
